About Transitioning to Work

Our Transitioning to Employment Program is designed to support members as they prepare for and succeed in the workplace. We focus on each person’s strengths and goals, offering customized services such as job skill development, resume help, interview practice, and job matching. Our team works closely with participants to make sure they are ready for employment and feel confident in their abilities.

In addition to job training, we provide ongoing support to help individuals stay successful in their roles. This includes vocational guidance, therapeutic services, and recreational activities that build confidence and independence. Whether someone is just starting to look for work or needs help keeping a job, our program offers the tools and support they need every step of the way.

The different types of activities vary per individual but some of the universal activities include arts & crafts, game room, community room, etc. 

What is the difference between the adult developmental home and the group home?

Group Homes for Adults with Developmental Disabilities:

Track House Life’s Group Home Program is tailored for individuals who require special assistance with daily living.

 

This program is a 24-hour per day service that provides for a safe and healthy living environment that meets the physical needs of the consumer. Track House Life also ensures that each home has furnishings and decor and any necessary modifications to optimize independence and personal preferences of our residence. Our goal is to ensure a safe and healthy environment, which meets the physical and emotional needs of the residents, as well as culturally appropriate with the preference of the residents. The individuals living in our homes participate in normal family home and community activities, which provide daily opportunities for learning and developing important life skills. Training is provided in the following areas:

  • Behavior management
  • Medication Management
  • Independent Living Skills
  • Community Access and Reintegration
  • Social Recreational Skills and
  • Self-Care

With our training, individuals develop the techniques and supports necessary to accommodate their residual cognitive, behavioral, and emotional difficulties in order to lead full and productive lives.

Adult Developmental Homes:

Adult developmental homes are family homes which provide supports and care for a person(s) with disabilities in a family environment. Residents would live in the home with his/her licensed caregiver(s) while receiving services to meet his/her individualized needs.

 

Track House Life provides supportive Developmental Home Services for adults with developmental disabilities. Our Adult developmental homes are designed to maximize the functioning of consumers. Each consumer will have a homelike environment that allows them a choice in their personal environment (space) and will be provided with nutritional balance in their diet. Each home will deliver a safe and healthy environment that meets the physical and emotional needs of the consumer. THL will establish, support and maintain licensed/certified developmental homes to meet the needs of the consumer. Provide monthly consultation and support. Support will include but not limited to respite relief, programmatic support and monthly home provider support groups.
